Job summary

Elara Caring is seeking a Physical Therapist to deliver compassionate, high-quality care to patients in their homes. You will provide direct patient care, evaluate functional needs, and develop plans of care with documentation of outcomes.

Qualifications

• Graduate from an APTA-approved PT program.
• Current, unrestricted PT license valid for Texas.
• Minimum one year of healthcare facility experience.
• Able and willing to travel within branch/office coverage area (50%).
• Must possess a valid driver’s license or reliable transportation.
• Must be able and willing to travel 50%.
• Ability to lift/move patients up to 50 pounds.

Responsibilities

• Provide direct patient care, evaluate functional needs and outcomes, and consult with other specialists.
• Design and implement a plan of care based on thorough assessment.
• Document evaluations, treatment goals, and plans; update documentation regularly.
• Direct staff to achieve patients’ functional goals.
• Promote physical therapy in the community; act as a resource for the field.
• Work in a physically demanding, high-stress environment.
• Possibly work irregular hours including call hours.
