Job summary

Adoration Health is seeking a Home Health Physical Therapist to provide patient-centered care in the Denver area with coverage across CO and TX. The role offers full-time hours, 8-5, Monday through Friday, and requires passion for helping patients in the home setting.

Responsibilities include evaluation, goal setting, documentation, and collaboration with the care team. Qualifications include a PT degree, valid license, CPR, and 2 years of experience preferred.

Qualifications

• Minimum of Bachelor’s Degree in Physical Therapy from an accredited PT program.
• New PT graduates may be considered in select markets based on program availability.
• At least 2 years of relevant PT experience; hospice/community experience preferred.
• Valid license in state of practice and current CPR certification.

Responsibilities

• Perform PT evaluation and treatment based on physician orders and procure adaptive equipment.
• Establish measurable goals and develop plans to move patients toward independence.
• Maintain communication among the treatment team, patient, and family.
• Maintain thorough and timely documentation for proper billing.
• Supervise PTAs and PT aides in direct patient care as per state regulations.
