Qualifications

• Graduate of a CAPTE approved Physical Therapist Education program.
• Current Physical Therapist licensure in State and CPR certification required.
• Two (2) years of experience in adult physical therapy, preferably home health.

• Perform physical therapy evaluation and treatment based on physician orders.
• Establish measurable goals and develop treatment plans.
• Maintain communication among treatment team, physician, patient, and family members.
