Job summary

Inspired Homecare is seeking a licensed Physical Therapist to join our home health team. You will perform comprehensive assessments, develop personalized treatment plans, and deliver high-quality care to patients in their homes.

Qualifications

• A degree in Physical Therapy from an accredited university/program.
• A current, unrestricted state license as a Physical Therapist in the state of practice.
• Demonstrated proficiency in clinical assessments, documentation, and compliance with policies and procedures.
• Strong organizational and communication skills.
• Experience in Home Health Therapy and completion of OASIS documentation is preferred.

Responsibilities

• Conduct comprehensive assessments and create personalized treatment plans for home health patients.
• Deliver high-quality care to individuals in the comfort of their homes.
