Inspired Homecare is seeking a Home Health Physical Therapist to deliver patient-centered care in patients' homes in Illinois. The role emphasizes comprehensive assessments, personalized treatment planning, and ongoing progress monitoring within a flexible, part-time schedule.

The ideal candidate holds a PT degree, an active state license, and strong organizational and communication skills. Experience with OASIS documentation is preferred to ensure quality, compliant care delivery.

Qualifications

• 1. A degree in Physical Therapy from an accredited university/program.
• 2. A current, unrestricted state license as a Physical Therapist.
• 3. Demonstrated proficiency in clinical assessments, documentation, and compliance with policies.
• 4. Strong organizational and communication skills.
• 5. Experience in Home Health Therapy and OASIS documentation preferred.

Responsibilities
