Job Summary: MercyOne Home Care in Des Moines is seeking a compassionate and dedicated Home Care Registered Nurse (RN) to join our team. You will provide high-quality nursing care to patients in their homes, supporting their health and independence.

Key Responsibilities:

• Conduct comprehensive assessments of patients' health status and needs.
• Develop and implement individualized care plans in collaboration with interdisciplinary teams.
• Administer medications and treatments as prescribed and monitor patient responses.
• Educate patients and families on disease management, medication usage, and health maintenance.
• Monitor vital signs and manage chronic conditions, ensuring timely interventions.
• Document patient care activities accurately and promptly.
• Coordinate care with physicians, therapists, and other healthcare professionals.
• Provide emotional support and counseling to patients and families.
• Ensure compliance with healthcare regulations and standards.
• Participate in quality improvement initiatives and ongoing education.

Qualifications:

• Current RN licensure in Iowa.
• Bachelor of Science in Nursing (BSN) preferred; ADN acceptable.
• Previous home health or community nursing experience preferred.
• Strong assessment and critical thinking skills.
• Ability to work independently and manage time effectively.
• Valid driver’s license and reliable transportation.
