AdventHealth in Ormond Beach, FL is seeking a licensed Physical Therapist to provide home health services. You will develop patient-specific plans, reassess progress, and educate patients and caregivers on home programs. The role requires the ability to coordinate with families and physicians and meet documentation standards.

Qualifications

• 1+ year of clinical physical therapy experience required.
• Experience in Medicare-certified home health care preferred.

Responsibilities

• Develop patient-specific care plans with measurable goals in collaboration with patients, families, and physicians.
• Reassess patients, document progress, and adjust plans of care.
• Educate patients and caregivers on use of assistive devices and home programs.
• Observe and report changes to physicians and coordinate care.

Location: We have need for PTs to cover Volusia and Flagler County

Mileage reimbursement provided: $0.76 per mile

Flexible scheduling with Full Time, Part Time and PRN available

• Other duties as assigned.
• Develops patient-specific care plans with realistic, measurable goals in consultation with patients, families, and physicians that Considers the physical, cultural, psychosocial, spiritual, age-specific, and educational needs of the patient.
• Reassesses patients at appropriate intervals, documents progress, and determines the effectiveness of therapy.
• Observes and records patient activities and findings, reporting any changes or deviations in the plan of care to the physician.
• Instructs patients and caregivers on the use of assistive devices and establishes home programs.

Knowledge, Skills, and Abilities:

• Ability to delegate tasks to appropriate personnel as indicated by skill level and professional standing
• Home Care Regulations and Third-Party Reimbursement as it impacts care delivery
• Demonstrated knowledge of physical therapy theory, modalities, treatments, and skills
• Basic computer skills and knowledge of Microsoft Office.
• Must be able to read, write, and speak conversational English.

Education:

• Doctorate [Preferred]

Field of Study:

• physical therapy

Work Experience:

• 1+ year relevant clinical physical therapy experience [Required]
• Recent, relevant experience in a Medicare-certified home health agency as a care-manager [Preferred]

Additional Information:

• Education requirements for this position are determined by the licensing or certification standards set forth by the relevant certifying body. Applicants must possess the level of education mandated for eligibility to obtain and maintain the required license or certification.

Pay Range:

$35.05 - $65.19
