You'll assess, treat, document, and coordinate care to support geriatric patients in regaining mobility and independence. Requirements include an accredited PTA degree, state licensure, valid driver's license, CPR certification, and ability to work in a team within home health settings.

Qualifications

• Associate degree in Physical Therapy (APTA).
• Current state license to practice Physical Therapy.
• Read, write, and speak fluent English.
• Valid driver's license, auto insurance and reliable transportation.
• Current CPR certification.
• Experience collaborating with a team of healthcare professionals.
• Geriatric population knowledge.

Responsibilities

• Administer the physical therapy treatment plan to patients in their homes, as developed by the Physical Therapist.
• Deliver non-complex active and passive manual therapy, modalities as prescribed.
• Document client records and submit on time each week.
• Attend educational meetings and coordinate department in-services.
• Notify supervisor of equipment failures or supply issues.
• Apply infection control and universal precautions in patient care.
