Job summary

CenterWell Home Health seeks a Home Health Physical Therapy Assistant to administer prescribed therapy in patients' homes, under the supervision of a Physical Therapist. You will document progress and participate in department in-services to ensure quality care.

Qualified candidates will hold a state PT license, CPR, and a valid driver’s license. Tuberculosis screening and driver safety rules apply as part of Humana’s program.

Qualifications

• Current state license to practice Physical Therapy.
• Valid driver's license, auto insurance and reliable transportation.
• Current CPR certification.
• Experience collaborating with a team of healthcare professionals.
• Comprehensive knowledge of geriatric physical, emotional, social and biological changes.

Responsibilities

• Administer the physical therapy treatment plan in patients' homes as prescribed by the PT.
• Deliver non-complex manual therapies and modalities as directed.
• Document client records and submit on time weekly.
• Attend educational meetings and coordinate department in-services.
• Report equipment or supply issues to PT or Branch Director.
• Follow infection control and universal precautions in patient care.
