AdventHealth Home Health and Hospice Inc is seeking a licensed Physical Therapy Assistant (PTA) for a full-time home health position based in Temple Terrace, FL. The role supports interdisciplinary care, adheres to PT/insurance guidelines, and requires weekend on-call rotation with a Monday–Friday schedule.

Preferred candidates have 1+ year home health experience; the position includes a sign-on if location allows, with standard benefits and comprehensive clinical support from AdventHealth.

Qualifications

• Associate degree as PTA.
• Licensed Physical Therapy Assistant (PTA) required.
• Basic Life Support (BLS) certification required.
• Driver's License and Auto Insurance required.

Responsibilities

• Maintain open communication with the case manager and other healthcare professionals to facilitate interdisciplinary continuity of care and discharge planning.
• Maintain productivity standards and adjust work hours to meet patient needs.
• Participate in departmental meetings, education sessions, and performance improvement initiatives to maintain competencies.
• Monitor patient response to treatment and coordinate with the Physical Therapist for adjustments.
• Educate patients, family members, and caregivers on identified needs.
• Document interventions and progression per policies and guidelines.

Job Description:

Up to a $5,000 Sign on Bonus Available (may vary by location) Mileage reimbursement provided: $0.76 per mile Schedule: Monday - Friday, 8am - 5pm; weekend on-call rotation Company provided phone and laptop/iPad Coverage Areas (one of the following): Hillsborough County Pasco County Pinellas County Polk County Sebring/Highlands/Hardee

• Maintains open communication with the case manager, primary therapist, and other healthcare professionals to facilitate interdisciplinary continuity of care and appropriate discharge planning.
• Maintains productivity standards as defined by the department and adjusts work hours to meet the needs of patients.
• Participates in departmental meetings, education sessions, and performance improvement initiatives to maintain clinical competencies and adhere to professional and evidence-based practices.
• Monitors patient response to treatment and communicates with the Physical Therapist for necessary adjustments.
• Educates patients, family members, and caregivers on identified needs, ensuring understanding.
• Implements treatment plans established by an Physical Therapist utilizing recommended techniques, adhering to insurance guidelines and patient needs.
• Documents interventions and patient progress accurately, completely, and timely, using measurable and objective terminology in line with standards of practice, insurance guidelines, and department/hospital policies and procedures.
• Other duties as assigned.

Knowledge, Skills, and Abilities:

• Entry level competency of Physical therapy practice, theory, principle, and applications as defined in competency department policies.
