Job summary

Enhabit Home Health & Hospice is seeking a licensed Physical Therapist to deliver skilled services in patients' homes, focusing on mobility and daily living activities. The role includes collaborating with the care team to implement plans of care and to monitor patient progress.

You will educate patients and families, participate in programs such as Balance and Fall Prevention, and work under the direction of the Branch Director to ensure high-quality, compassionate care.

Qualifications

• Must possess valid state driver's license.
• Must possess automobile liability insurance.
• Must wear seatbelts at all times while driving.
• Must have dependable transportation kept in good working condition.
• Must be licensed in the state of employment.

Responsibilities

• Examine and treat patients to improve mobility and daily living.
• Provide skilled therapy services under physician orders and plan of care.
• Educate families and health care team members on treatment plans.
