Job summary

PruittHealth is seeking a licensed Physical Therapist to provide evaluative, consultative, and therapeutic interventions in patients’ homes in the Black Mountain area, NC. You will communicate with patients, families, and interdisciplinary teams to support optimal outcomes.

The role emphasizes adherence to a plan of care, documentation accuracy, and participation in continuous improvement initiatives within a dynamic home health team.

Qualifications

• Must have a valid professional license in State of practice.
• Graduate of an accredited Physical Therapy Program.
• If foreign-trained, the program must be deemed substantially equivalent to a Physical Therapist entry level education in the U.S. by a credentials evaluation organization approved by the APTA.

Responsibilities

• Evaluate the patient’s function and skilled therapy needs.
• Treat the patient to relieve pain, develop or restore function, and obtain maximum outcomes.
• Advise and consult the family and team members to promote optimal functional outcomes.
• Document progress and patient health using required records and timelines.
• Collaborate with the care team to develop and manage the plan of care.
