Job summary

Peoples Health Services is seeking a dedicated Physical Therapist to provide medically prescribed home-based therapy across our Florida Panhandle service area. You will develop and implement individualized plans of care, supervise as needed, and collaborate with PTA according to state guidelines and agency policies.

Responsibilities include home visits, patient and family education, progress evaluations, and active participation in interdisciplinary team meetings.

Qualifications

• Current Florida Physical Therapist license required.
• CPR certification required.
• Valid driver's license with auto and required insurance.

Responsibilities

• Performs home-based physical therapy per physician prescription and plan of care.
• Teaches exercises to patients and families and translates exercises into daily living activities.
• Maintains patient records and completes OASIS assessments when appropriate.
• Coordinates with the interdisciplinary team for discharge planning and program updates.
• Ensures patient confidentiality and professional image while in patients' homes.
