Job summary

Peoples Home Health, LLC is seeking a licensed Physical Therapist to provide medically prescribed therapy in patients' homes within the Florida Panhandle. You will develop and implement plans of care, supervise PT support staff as needed, and ensure quality documentation.

The role requires direct patient care, ability to coordinate with physicians and the care team, and commitment to patient-centered outcomes while maintaining confidentiality and professional standards.

Qualifications

• Current Florida Physical Therapist License.
• CPR certification.
• Current driver’s license with access to an automobile and required insurance coverage.

Responsibilities

• Performs physical therapy procedures, evaluations, and goals in patients’ homes.
• Teaches treatment procedures to patients and families.
• Translates exercises into activities of daily living and modifies home equipment as needed.
• Completes OASIS assessments at appropriate time points.
• Participates in interdisciplinary team conferences and quality improvement processes.
