Job summary

UnitedHealth Group is hiring a full-time Physical Therapist to join the Longwood Home Health team in Florida. A $15,000 sign-on bonus is offered for full-time hires.

The PT will assess patients, develop and implement treatment plans, and coordinate with physicians and nurses. Responsibilities include evaluating functional status, providing therapy services under state law, documenting progress, and communicating care plans to patients and families.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver’s license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Preferred: CPR certification or ability to complete within 90 days of hire.

Responsibilities

• Evaluates the patient's functional status and physical therapy needs.
• Provides services within the scope of practice and plan of care.
• Observes and reports patient response to treatment and changes in condition.
• Treats patients to relieve pain, develop and restore function, and maintain maximum functioning.
• Instructs patients and family on plan of care, limitations, and progress.
• Visits patients per Plan of Treatment and submits progress notes.
• Develops PT plan of care within state guidelines and reports outcomes.
