Job summary

Covenant Health HomeCare in Knoxville is seeking a Physical Therapist to provide rehabilitation services at patients' homes. You will assess function, design and adjust treatment plans, and work with the care team to maximize outcomes and independence.

Local travel across the service area is required and you will follow Medicare and Covenant Health policies. Minimum qualifications include graduation from an accredited physical therapy program and two years’ home health or acute care experience;

Qualifications

• Minimum Education: Graduation from an accredited program of physical therapy.
• Minimum Experience: Staff Physical Therapist must have two (2) years’ experience in home health or an acute hospital setting. Clinical Specialist must have a minimum of four (4) years experience with three (3) years in area or specialty.

Responsibilities

• Provides patient care including initial and ongoing evaluations to determine level of function.
• Develops and revises Plan of Care in consultation with physician.
• Sets goals and plans treatment in consultation with care-team members.
• Documents evaluation data, treatments, and patient responses.
• Educates and consults with clients, families, and agency staff.
• Supervises physical therapy assistants and home health aides as appropriate.
• Conducts training for medical or nursing staff on physical therapy topics.
• Assists with onboarding of support staff and development of PT students.
• When assigned to hospice patients, participates in goal setting and care planning with the interdisciplinary team.
