Responsibilities

The Physical Therapist (PT, DPT, RPT) in Home Health assesses and evaluates patient needs related to functional status, mobility, endurance, pain, and other medical conditions. Based on this, they develop treatment plans, perform interventions to improve patient well-being, and evaluate progress.

• Evaluate patient's functional status and therapy needs; assist in developing the plan of care with the physician
• Monitor and report patient responses and changes to the supervising nurse or physician
• Perform treatments to relieve pain, restore function, and maintain maximum functioning according to standards of practice
• Educate patients, families, and healthcare team members about the plan of care, limitations, and progress

Education and Experience

Licensure Requirements

• Current Physical Therapy license in the state of Maryland
• Current CPR certification
• Valid driver's license, vehicle insurance, and access to reliable transportation
