Responsibilities

The Physical Therapist (PT, DPT, RPT) in Home Health assesses and evaluates patient needs related to mobility, endurance, pain, and other medical conditions. They develop treatment plans, perform interventions to improve patient well-being, and monitor progress.

Responsibilities include:

• Evaluating patient functional status and therapy needs
• Assisting physicians in developing the therapy plan of care
• Observing and reporting patient responses and condition changes
• Treating patients to relieve pain, restore function, and maintain maximum functioning
• Educating patients, families, and healthcare team members about the plan of care and progress

Education and Experience

Licensure Requirements:

• Current Physical Therapy licensure in the state of practice
• Current CPR certification
• Valid driver's license, vehicle insurance, and access to a dependable vehicle
