Job summary

ProHealth Home Health is seeking a Registered Physical Therapist for PRN home health visits in Mesquite/Forney, TX. The PT administers therapy in patients' homes under physician orders and clinical supervision.

Requirements include graduation from an approved PT program, Texas licensure, and the ability to exercise initiative. Two years’ experience is preferred, with a focus on returning patients to their prior functional level.

Qualifications

• Graduate from a physical therapist curriculum approved by relevant bodies.
• Currently licensed in Texas.
• Two years’ experience preferred.
• Acceptance of philosophy and goals of this Agency.
• Ability to exercise initiative and independent judgment.

Responsibilities

• Understands and adheres to established policies and procedures.
• Provides physician prescribed physical therapy.
• Improves or minimizes residual physical disabilities.
• Returns patient to optimum, productive level within capabilities.
• Participates in care planning with home care personnel.
• Directs and supervises personnel as required.
• Takes initial history and evaluation.
• Performs skilled procedures as ordered by a physician.
• Consults with physicians regarding treatment changes.
• Writes progress reports to physicians.
