Western CT Home Care Inc is seeking a licensed Physical Therapist to provide home health PT services within the designated Connecticut region. You will assess functional limitations, develop and follow the plan of care, and coordinate with physicians and the care team to support patient independence.

Responsibilities include documenting assessments, treatments, and patient progress, along with educating patients and caregivers.

Qualifications

• Bachelor's level degree required.
• Minimum one year experience in home health setting preferred.
• Licensed PT in CT.

Responsabilités

• Performs physical therapy assessment including functional limitations, mobility, endurance, ADL status.
• Identifies patient and family needs for other home health services and refers for same when needed.
• Provides services ordered by physician as indicated in the plan of care and communicates with involved providers.
• Treats patients to relieve pain, develop and restore function, and regain independence using appropriate modalities.
• Records evaluation data, treatments, and client response on a visit basis and plans discharge.
• Participates in case management conferences to coordinate therapy and total plan of care.
• Counsels and educates patients, family, and other health care personnel on treatment regimen with proper documentation.
• Provides equipment as needed to increase function and independence.
• Participates in QA and inservice training as required by the agency.
• Supervises PTA staff and home health aides as appropriate.
• Fulfills all compliance responsibilities related to the position.
