Job summary

UnitedHealth Group is seeking a Physical Therapist for home health in Colorado. You will perform assessments, develop a therapy plan, implement interventions to improve well-being, and evaluate progress.

Responsibilities include evaluating functional status, coordinating with the care team, educating patients and families, and documenting visits and outcomes. A current PT license and reliable transportation are required; CPR is preferred.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver’s license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Preferred: CPR certification or ability to complete within 90 days of hire.

Responsibilities

• Evaluates the patient's functional status and therapy needs.
• Assists the physician in developing the therapy plan of care.
• Provides PT services within the scope of practice as defined by state laws and the plan of care.
• Observes and reports the patient's response to treatment and any changes in condition to the supervising nurse or physician.
• Treats patients to relieve pain, develop and restore function, or maintain maximum functioning.
• Instructs patient and family/caregivers regarding the plan of care and progress toward goals.
• Visits patients according to Plan of Treatment and submits progress notes per policy.
• Completes all patient evaluations and develops the PT plan of care within state guidelines.
