Qualifications

• Current Physical Therapy licensure in state of practice.
• Current CPR certification.
• Access to dependable vehicle or public transportation.

Responsibilities

• Evaluates the patient’s functional status and therapy needs and assists in development of the plan of care.
• Provides services within the scope of practice per the plan of care and in coordination with the health care team.
• Observes and reports patient responses to treatment and changes to supervising staff.
• Treats patients to relieve pain and restore or maintain function.
• Instructs patient, family, and care team regarding plan of care and progress.
• Visits patients per Plan of Treatment and submits progress notes.
• Completes all evaluations and develops the PT plan of care within state guidelines.
• Reports outcomes of evaluation, goals, and frequency of care.
