Job summary

Atrinity Home Health is seeking a dedicated Physical Therapist to perform assessments, develop and implement treatment plans, and collaborate with physicians and the healthcare team to improve patient outcomes. You will evaluate mobility and functional status, determine care plans, and document progress in the patient’s chart.

Qualifications

• Must hold current Physical Therapy licensure in state of practice.
• Current CPR certification is required.
• Valid driver's license and access to a vehicle or reliable transportation.

Responsibilities

• Evaluates patient functional status and physical therapy needs.
• Develops and implements therapy plan of care.
• Observes patient response and reports changes to supervising nurse/physician.
• Treats patients to relieve pain and restore function.
• Educates patient and family about plan of care and progress.
• Completes progress notes and PT plan of care within state guidelines.
• Reports outcomes and anticipated frequency of care.
