Atrinity Home Health, part of LHC Group and united under the UnitedHealth Group family, is hiring a Physical Therapist for home health services in the United States. The role involves patient assessments, treatment planning, and interventions to improve well-being and mobility.

You will work with the health care team to deliver care in patients’ homes and evaluate progress over time. The position emphasizes comprehensive patient care, collaboration with physicians, and adherence to state PT

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current CPR certification.
• Current driver's license and access to dependable transportation.
• Ability to function in any home situation regardless of age, race, creed, color, sex, or financial condition.

Responsibilities

• Evaluates patient functional status and PT needs; assists in developing the therapy plan of care.
• Provides PT services within scope of practice and plan of care.
• Observes and reports patient response to treatment to supervising nurse/physician.
• Treats patients to relieve pain and restore function; maintains maximum functioning.
• Instructs patient and family/caregivers regarding plan of care and progress toward goals.
• Visits patients per Plan of Treatment; submits progress notes per policy.
• Completes patient evaluations and develops the PT plan within state guidelines.
• Reports outcomes of evaluation and anticipated frequency of care.
