Job summary

The Home Care Physical Therapist at Carle Health evaluates and develops treatment plans for patients of all ages, performing comprehensive assessments in patients’ homes and coordinating with physicians, insurers and care teams. The role requires documenting visit details and creating a clear plan for the next contact.

You’ll supervise assistants, administer physical therapy treatments such as heat, cold, electrical modalities, and fit prosthetic/orthotic devices as needed.

Qualifications

• Bachelor's Degree in Physical Therapy required
• Licensed Physical Therapist in Illinois or eligible
• BLS certification within 30 days
• Valid Driver's License - Illinois
• Compliance with home health regulations and documentation standards

Responsibilities

• Document patient visits with clear plan for the next contact.
• Educate and document discharge planning prior to discharge with physician and insurer communication.
• Update goals promptly and adjust as patient progresses.
• Communicate and coordinate with care team; schedule and supervise assistants.
• Administer physical therapy treatments and assess prosthetic/orthotic needs as appropriate.
