Job summary

St. Elizabeth's Health at Home is seeking a skilled Physical Therapist to provide home-based PT services that promote rehabilitation and improve quality of life.

You will assess patients, develop personalized treatment plans, and collaborate with families and care teams to support recovery. Requirements include a Bachelor’s degree in Physical Therapy, an active PT license in Kentucky, CPR/BLS certification, and a valid driver’s license.

Qualifications

• Bachelor's degree in Physical Therapy from an American Physical Therapy Association approved program.
• Active PT license in the practicing state.
• Current CPR/BLS certification.
• One year PT experience preferred but not required.
• Current driver's license.
• Compliance with federal, state and local laws.

Responsibilities

• Conduct evaluations of patients’ physical abilities, mobility, strength, and functional limitations.
• Implement therapeutic exercises, manual therapy techniques, and modalities to improve patients’ function and mobility.
• Educate patients and their families about exercises, safety, and lifestyle modifications to promote recovery and prevent re-injury.
• Monitor and document patient progress, adjusting treatment plans to achieve optimal outcomes.
• Maintain accurate patient records in compliance with regulations and policies.
