Job summary

JourneyCare Home Health is seeking a skilled Home Health Physical Therapist to cover Dolton IL and surrounding areas. You will provide individualized PT services in patients’ homes, helping recover from injuries, surgeries, or manage chronic conditions.

The role includes planning and implementing effective treatment plans to improve mobility and quality of life. The position requires an approved PT education, active state licensure, CPR/First Aid, and reliable transportation.

Qualifications

• Graduate from a School of Physical Therapy approved by the APTA and related bodies.
• Current licensure as a Physical Therapist in the state of practice.
• At least two years of experience in acute hospital or equivalent preferred.
• Must possess CPR and First Aid Certification.
• Must have reliable transportation and valid driver’s license.

Responsibilities

• Provide PT services to home patients per physician care plan.
• Explain and demonstrate treatment procedures to clients and family.
• Asses function and progress; adjust plan of care as needed.
• Prepare clinical notes and progress summaries.
• Supervise assistants per policy and state regulations.
• Maintain accurate records of goals and patient progress.
