• Minimum 2 years home health experience.
• Ability to work in patients' homes and coordinate with families.

Responsibilities

• Evaluation and assessment of patients to identify impairments and plan treatment.
• Develop individualized treatment plans with exercises, manual therapy, and modalities.
• Provide hands-on therapy to improve strength, balance, and mobility.
• Educate patients and families on safe exercises and body mechanics.
• Document assessments, plans, progress, and outcomes in EHR.
• Collaborate with physicians, nurses, and therapists for coordinated care.
• Adhere to safety, infection control, and regulatory requirements.
