Job summary

Penn Medicine seeks a Physical Therapist to deliver skilled therapy in patients' homes, assessing mobility and function, relieving pain, and preventing disability. You will design and implement individualized treatment plans to restore function and maximize patient independence.

You will collaborate with the interdisciplinary team, schedulers, and department managers to ensure a seamless patient experience and to meet evolving care needs across a home-health setting.

Qualifications

• Bachelor's degree from CAPTE accredited school of physical therapy (MS or Doctorate preferred).
• Licensed to practice physical therapy in Pennsylvania.
• BLS/CPR as a healthcare provider per the American Heart Association.
• Valid Driver's License.
• Car Insurance.

Responsibilities

• Provide skilled therapy to patients in their homes, assessing physical abilities and restoring maximum function while relieving pain and preventing disability.
• Collaborate with interdisciplinary team, schedulers, and managers to ensure optimal patient experience and design individualized PT programs.

Education
