Job summary

Penn Medicine seeks a licensed Physical Therapist to provide skilled therapy to patients in their homes, assessing physical abilities and helping restore maximum function. You will develop and carry out individualized treatment plans to maintain or improve physical functioning and reduce pain.

You will collaborate with an interdisciplinary team to optimize the patient experience and meet patient needs in a supportive home-care setting.

Qualifications

• Bachelor's degree from CAPTE accredited school of physical therapy (MS or Doctorate preferred).
• Licensed to practice physical therapy in Pennsylvania.
• BLS/CPR certification as a healthcare provider.

Responsibilities

• Provides skilled therapy to patients in their homes, assessing physical abilities and restoring maximum function.
• Plans and implements individually designed physical therapy programs to maintain or improve functioning and alleviate pain.
• Collaborates with an interdisciplinary team to ensure optimal patient experience and meet patient needs.
