Concierge Home Care in Kissimmee, FL is seeking a Home Health Physical Therapist (PT) to provide 1:1 patient care in the home. The role offers a flexible schedule, autonomy, and the opportunity to increase income based on productivity.

You will evaluate patients, develop individualized treatment plans, and work with physicians and interdisciplinary teams. This position supports growth with mentorship, structured onboarding, and access to WellSky/Kinnser EMR.

• Average modified full-time earnings: $95,000 – $120,000+

Your Role as a Home Health Physical Therapist

• Evaluate patients and develop individualized treatment plans
