Classic Home Care in Michigan is seeking dedicated Home Health Physical Therapists (PTs) to join our nurse-owned home health agency. We offer contingent PT positions with flexible hours, potential to transition to full-time or part-time caseloads over time.

Compensation includes per-visit rates with mileage reimbursement, the option for non-taxed mileage, orientation hours, and competitive benefits for part-time and full-time roles.

Qualifications

• Active Michigan Physical Therapy license.
• Reliable transportation and auto insurance in your name.
• Home health care experience preferred.

Responsibilities

• Provide skilled physical therapy per physician's plan of care.
• Asses patient functional level and contribute to plan of care and goals.
• Treat patients to relieve pain and restore function.
• Observe and report patient responses and changes in condition to physician and care team.
• Instruct patient and family in therapy techniques and use of equipment.
• Participate in in-service programs and collaborate with home health aides and PT assistant.

Contingent Home Health Physical Therapist (PT) Compensation:

• Visit Rates with Mileage Reimbursement: $63.00 - $100.00 per visit
• Visit Rates without Mileage Reimbursement: $68.00 - $105.00 per visit
• Discharge Without a Visit: $5.00 - $10.00 per non-visit discharge
• Transfer Without a Visit: $10.00 per non-visit transfer
• Orientation and In Service Hours: $30.00 per hour
• Non taxed mileage reimbursement is paid at $0.50 per mile after 30 miles per day

Job Description:

• Provides skilled physical therapy in accordance with the physician's plan of care. Accepts only patients referred by the patient care manager or designee.
• Assists the physician in assessing the patient's functional level by applying appropriate tests and assists in the development and revision of the plan of care, including OASIS assessments and goal setting as appropriate.
• Treats patients to relieve pain, develop and restore function and maintain maximum performance, using appropriate physical therapy treatments, modalities and procedures as stated in the physical therapy plan of care. Identifies barriers to desired outcomes.
• Observes, records and reports the patient's reaction to treatment and any change in the patient's condition to the physician and/or patient care manager and other staff involved.
• Instructs and advises patient, family and other health team personnel including, when appropriate, the physical therapy assistant and home health aides, in the phases of physical therapy in which they may assist the patient; instructs all of the above in the use of prosthetic and orthotic devices and durable medical equipment, as appropriate. The goal is to teach and promote disease prevention and injury management for eventual patient & caregiver independence, utilizing agency materials as appropriate.

Requirements:

• Active Michigan Physical Therapy license
• Reliable transportation
• Valid driver's license and auto insurance in your name as a driver
• Home health care experience preferred

Degree of Travel:

• Home visits and office meetings as indicated. Must participate in necessary in-service programs. Must have reliable transportation and agency-required auto insurance.
