At Aveanna, we are seeking a dedicated Physical Therapist responsible for planning, conducting, and implementing physical therapy treatment programs in accordance with medical referrals and evaluations. The goal is to restore or maintain patient wellbeing while adhering to confidentiality standards and professional boundaries.

Responsibilities

1. Deliver physical therapy services according to physician prescriptions.
2. Follow policies, procedures, and comply with all patient care standards, licensure regulations, and state laws.
3. Provide written evaluations outlining physical therapy needs, goals, progress, and outcomes in collaboration with the referring physician’s Plan of Treatment.
4. Coordinate services with healthcare team members, including discharge planning.
5. Maintain effective communication with staff, healthcare team, and family.
6. Serve as a resource for clinical staff.
7. Supervise other personnel (PTA, HHA) as applicable.
8. Complete OASIS assessments where permitted by state regulations.

Requirements

1. Current, unrestricted state license as a Physical Therapist.
2. Current CPR certification.
3. Proficiency in clinical assessments, documentation, and compliance with policies.
