Position Overview
Aveanna Healthcare has grown tremendously during the past year through a series of exciting acquisitions and we are currently looking for a Physical Therapist (PT) to join our dynamic home health care team. In this position, you make it possible for patients to recuperate from illness or injury in the comfort of their own home.
Essential Job Functions
- Perform initial and ongoing assessments to evaluate mobility, coordination, strength and current level of function.
- Responsible for administration, evaluation, and interpretation of tests and measurements to determine functional goals and outcomes.
- Provide a written evaluation and plan of care defining physical therapy needs, goals, progress and expected outcomes.
- Deliver physical therapy services in accordance with physician orders.
- Maintain plan of care through timely documentation, goal modification, and discharge planning.
- Collaborate with patients and caregivers on home exercise plans post-discharge.
- Coordinate services with healthcare team members, including physicians, nurses, and other staff.
- Document according to policies and procedures, requiring minimal supervision.
- Ensure documentation is thorough to prevent denials upon payer review.
- Maintain effective communication with staff, healthcare team, and family members. Serve as a resource for clinical staff.
- Supervise other personnel (PTA, HHA) as applicable.
- Complete OASIS assessments where permitted by state regulations.
- Adhere to all policies, procedures, licensure requirements, and state laws.

Requirements
- Degree in Physical Therapy from an accredited university/program.
- Current, unrestricted state license as a Physical Therapist in the state of practice.
- Proficiency in assessments, documentation, and compliance with policies.
Preferred Skills
- Strong organizational and communication skills.
- Attention to detail and time management.
- Ability to build relationships.
- Experience in Home Health Therapy and OASIS documentation.
