Job summary

Hill country Preferred Senior Care Home Health in Mason, TX is seeking a PRN/Pay Per Visit Physical Therapist to join our home health team. You will assess patients, develop and implement plans of care, and supervise therapy sessions in patients' homes.

The role focuses on rehabilitation, gait training, ADLs, and discharge planning. Requires TX PT license, APTA-approved program, and driving with insurance. Flexible hours and community-based care offered.

Qualifications

• Graduate of an American Physical Therapy Association approved physical therapy program.
• Current license to practice physical therapy in Texas.
• Current driver's license with current automobile insurance.

Responsibilities

• Conducts patients' assessments and assists the physician in evaluating the level of function by applying diagnostic and prognostic functional ability tests.
• Assist in the development of the Plan of Care.
• Directs and aids patients in active and passive exercise, muscle re-education, gait, functional, ADL, transfer, and prosthetic training.
• Treats patients to relieve pain; develop and restore function; maintain maximum functioning using exercise, massage, heat, water, light, and electricity.
• Observes, records, and reports patient responses to treatment to the physician.
• Instructs patients and caregivers in care and use of devices; participates in staff conferences and education.
