Job summary

HCA Houston Healthcare at Home in Wisconsin is seeking a Home Health Physical Therapist PRN to provide high-quality rehabilitation in patients' homes as part of our national healthcare network.

You will assess patient needs, develop personalized treatment plans, deliver evidence-based interventions, and collaborate with physicians and the care team to restore mobility and independence, while mentoring students and ensuring thorough documentation.

Qualifications

• Licensed Physical Therapist (LPT) with current credentials.
• Driver License (DL) required.
• Bachelors Degree required.

Responsibilities

• Evaluating patient abilities and developing personalized treatment plans with physicians and the care team.
• Delivering evidence-based interventions to restore movement and reduce pain.
• Monitoring progress and updating documentation of outcomes.
• Educating patients and families on exercise programs and safe practices.
• Collaborating across disciplines to ensure cohesive care and smooth transitions.
• Leading by example and mentoring therapy assistants or students.

Your role will include:
