Job summary

PARS Therapy is seeking a compassionate Physical Therapist to join our home health team in the Kingwood and Humble, TX area. This onsite role focuses on evaluating patients in their homes, creating personalized treatment plans, and guiding patients toward greater independence.

The ideal candidate will hold a TX PT license, have home health or rehab experience, and demonstrate strong communication with patients and caregivers while delivering evidence-based care.

Qualifications

• Active Physical Therapist license in TX.
• Professional liability insurance coverage for TX PT.
• Graduate degree in Physical Therapy from an accredited program.
• Experience in home health or rehabilitative settings preferred.
• Ability to adapt care approaches to diverse patients.
• Up-to-date knowledge of evidence-based techniques and best practices.
• Excellent communication skills and professional demeanor.

Responsibilities

• Evaluate patients’ physical abilities in a home health setting.
• Develop and implement individualized treatment plans to restore function.
• Provide interventions including exercise, gait training, manual therapy, balance retraining.
• Educate patients and caregivers on home exercise programs and safety.
• Monitor progress and adjust plans to achieve outcomes.
• Document evaluations, sessions and progress notes on digital platforms.
• Collaborate with physicians, nurses and occupational therapists.
• Conduct home safety assessments and prevent falls or re-injury.
• Deliver compassionate care to empower patient independence.

Essential Job Functions:

• Evaluate patients’ physical abilities and functional limitations in a home health setting
• Develop and implement individualized treatment plans to restore function, improve mobility, and alleviate pain
• Provide therapeutic interventions including exercise, gait training, manual therapy, and balance retraining
• Educate patients and caregivers on home exercise programs, safety techniques, and adaptive strategies
• Monitor patient progress and adjust plans as necessary to achieve optimal outcomes
• Accurately document evaluations, treatment sessions, and progress notes using digital platforms
• Collaborate with interdisciplinary team members including physicians, nurses, and occupational therapists
• Conduct home safety assessments and make recommendations to prevent falls or re-injury
• Deliver compassionate care while empowering patients to achieve greater independence

Requirements:
