The Physical Therapist in Home Health is responsible for assessing and evaluating patient care needs related to functional status, mobility, endurance, pain, and other medical conditions. Based on this assessment, the therapist develops a treatment plan, performs interventions to improve patient well-being, and evaluates progress.


Responsibilities

  • Evaluate the patient's functional status and physical therapy needs, and consult with physicians to develop the plan of care.
  • Provide services within the scope of practice defined by state laws, in accordance with the plan of care, and in coordination with the healthcare team.
  • Monitor and report patient responses to treatment and any condition changes to supervisors or physicians.
  • Treat patients to relieve pain, restore or develop function, and maintain maximum functioning using evidence-based practices.
  • Educate and inform the healthcare team, patients, and families about the plan of care, limitations, and progress.
  • Visit patients across the service area and nearby agencies as needed.
  • Complete and submit progress notes for each visit, and evaluate patient outcomes as per policy.
  • Develop the PT plan of care, report outcomes, goals, and projected care frequency within 24 hours of evaluation.
  • Participate in staff meetings and committees as requested.
  • Supervise home health aides and PTAs according to policy and regulation.
  • Participate in weekend rotations to provide services as needed.
  • Prepare discharge summaries and periodic patient condition reports.
  • Provide in-service education upon request.
  • Adhere to therapy standards of practice and maintain up-to-date clinical knowledge and licensure.
  • Perform other duties as assigned.

Education and Experience

Licensure Requirements

  • Current Physical Therapy licensure in the state of practice
  • Current CPR certification
  • Valid driver's license, vehicle insurance, and access to a reliable vehicle
