Home Health Physical Therapist – Per Diem

Job Location: Kanawha and Putnam counties, WV

Position Summary:

• Provides physical therapy services as ordered by the physician for Home Health of WV.
• The Physical Therapist (PT) provides consultation to the clinical team.
• Performs other related duties as required.

Position Qualifications:

• Graduate from an approved school of physical therapy.
• Current unrestricted WV license to practice as a Physical therapist.

Physical Demands / Work Environment:

• Ability to lift, transfer, and reposition patients up to 50 pounds independently and over 50 pounds with assistance.
• Frequent standing and walking for extended periods (8–12 hour per shift).
• Frequent bending, stooping, kneeling, crouching, and reaching.
• Ability to push and pull equipment (e.g., wheelchairs, stretchers, and medication carts).
• Visual acuity to read medication labels, physician orders, and monitors.
