Job summary

Adoration Health is seeking a Home Health Physical Therapist in Bluefield, WV. This part-time opportunity offers 1:1 patient care in Mercer and McDowell Counties, with a sign-on bonus for external candidates and comprehensive benefits.

You will evaluate and treat patients, set goals, coordinate with the care team, document thoroughly for billing, and supervise support staff as required. Licensure in WV and CPR certification are required.

Qualifications

• Bachelor's Degree in Physical Therapy from an accredited program.
• New PT graduates may be considered in select markets based on program availability.
• At least 2 years of relevant PT experience; hospice/community experience preferred.
• Valid license in the state of practice.
• Current CPR certification.

Responsibilities

• Perform physical therapy evaluation and treatment of patients based on physician orders, including adaptive equipment recommendations.
• Establish measurable goals and treatment plans to move patients toward maximum independence and function.
• Maintain communication among the treatment team, physician, patient, and family as applicable.
• Maintain thorough and timely documentation of services for proper billing.
• Supervise PT Assistants and PT Aides in direct patient care in accordance with state practice act.

• Establish measurable goals and develop treatment plans to move patients toward their maximum level of independence and/or function
• Maintain communication among the treatment team, physician, patient, and family members as applicable
• Maintain thorough and timely documentation of services provided to ensure proper billing
• Supervise Physical Therapist Assistants and Physical Therapy Aides in direct patient care and patient-related activities in accordance with state practice act

Qualifications

• Minimum of Bachelors Degree in Physical Therapy from a college or university with accredited Physical Therapy program
• New physical therapy graduates may be considered in select markets based on program availability
• At least (2) years of appropriate experience as a physical therapist. Community/hospice experience is preferred
• Valid license form the state of practice
• Possess and maintains current CPR certification
