Job summary

Adoration Home Health in Wheeling, WV is seeking a dedicated Home Health Physical Therapist (PT) to evaluate and treat patients in their homes. You will develop plans to maximize independence, while coordinating with physicians, patients, and families to ensure quality care.

The role offers PRN scheduling, with opportunities for growth within a supportive team and a stable employment environment that values empathy and patient-centered care.

Qualifications

• Minimum of Bachelor’s Degree in Physical Therapy from an accredited program.
• New graduates may be considered in select markets based on program availability.
• At least 2 years of appropriate experience as a physical therapist; hospice/community experience preferred.

Responsibilities

• Perform PT evaluation and treatment per physician orders, including adaptive equipment.
• Set measurable goals and develop treatment plans for independence.
• Maintain communication among treatment team, physician, patient and family as applicable.
• Maintain thorough and timely documentation for proper billing.
• Supervise PTAs and PT Aides in direct patient care per state act.

Responsibilities

• Perform physical therapy evaluation and treatment of patients based upon physician orders, including recommendation and procurement of adaptive equipment
• Establish measurable goals and develop treatment plans to move patients toward their maximum level of independence and/or function
• Maintain communication among the treatment team, physician, patient, and family members as applicable
• Maintain thorough and timely documentation of services provided to ensure proper billing
• Supervise Physical Therapist Assistants and Physical Therapy Aides in direct patient care and patient-related activities in accordance with state practice act

Qualifications

• Minimum of Bachelors Degree in Physical Therapy from a college or university with accredited Physical Therapy program
• New physical therapy graduates may be considered in select markets based on program availability
• At least (2) years of appropriate experience as a physical therapist. Community/hospice experience is preferred
• Valid license from the state of practice
• Possess and maintains current CPR certification
