Job summary

UnitedHealth Group is hiring a Physical Therapist for home health in Oregon. You will evaluate patients, develop and implement therapy plans, and work with a care team to improve function and reduce pain.

Responsibilities include conducting assessments, documenting progress, and coordinating with physicians and nurses. A valid Oregon PT license and reliable transportation are required; CPR is preferred and a 10,000 sign-on bonus is available for full-time eligible candidates.

Qualifications

• Current Physical Therapy licensure in state of Oregon.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• CPR certification preferred (within 90 days of hire).

Responsibilities

• Evaluates the patient’s functional status and physical therapy needs.
• Assists the physician in the development of the therapy plan of care.
• Provides services within the scope of practice per state laws and plan of care.
• Observes and reports patient response to treatment and changes in condition.
• Treats patients to relieve pain, develop and restore function, or maintain maximum functioning.
• Instructs patient and family on plan of care and progress toward goals.
• Visits patients per Plan of Treatment and submits progress notes.
• Completes patient evaluations and develops PT plan within state guidelines.
• Reports outcomes, goals, and anticipated frequency of care.

Education

Physical Therapy License in Oregon

CPR Certification (preferred)
