Your Role and Responsibilities
  • Evaluate patients' functional status and physical therapy needs, assisting physicians in developing the therapy plan of care
  • Provide services within the scope of practice as defined by state laws, following the plan of care, and collaborating with the healthcare team
  • Observe and report patient responses and condition changes to the supervising nurse or physician
  • Treat patients to relieve pain, restore or develop function, and maintain maximum functioning standards
  • Educate the healthcare team, patients, and families about the plan of care, limitations, and progress
  • Visit patients as per the Plan of Treatment, document each visit, and submit progress notes according to policy
  • Complete evaluations and develop PT plans within state guidelines
  • Report outcomes, goals, and projected care frequency

Licensure and Certification Requirements
  • Current Physical Therapy licensure in the state of practice
  • Current CPR certification
  • Valid driver's license, vehicle insurance, and access to a dependable vehicle
