Job summary

Celebrity Home Health, Inc. is seeking a licensed Physical Therapist to provide skilled evaluation and treatment in patients' homes. You will develop individualized plans of care, deliver therapy, and collaborate with an interdisciplinary team to improve mobility and independence.

Responsibilities include accurate EHR documentation, adherence to Medicare CoPs, clinical progress notes, and education for patients and families. Travel within the assigned territory is required.

Qualifications

• Active Nevada Physical Therapist license is required.
• Graduate of an accredited Physical Therapy program.
• Minimum 1 year of clinical experience (home health preferred).
• Strong knowledge of rehabilitation techniques and patient care standards.
• Excellent communication and interpersonal skills.
• Ability to work independently and manage time effectively.

Responsibilities

• Perform initial and ongoing physical therapy evaluations.
• Develop and implement individualized plan of care (POC) based on patient needs.
• Provide therapeutic exercises, manual therapy, and functional training.
• Assess patient mobility, strength, balance, and coordination.
• Modify treatment plans based on patient progress and outcomes.
