Job summary

Enhabit Home Health & Hospice seeks a dedicated Physical Therapist to evaluate and treat patients in the home, using modalities to improve mobility and daily living activities. You will participate in specialty programs such as Balance and Fall Prevention and Joint Replacement initiatives, delivering skilled care under physician orders and the plan of care.

You will also educate patients and families and coordinate with the health care team, operating under the supervision of the Branch Director

Qualifications

• Must possess a valid state driver's license.
• Must possess automobile liability insurance.
• Must wear seatbelts at all times while driving.
• Must have dependable transportation kept in good working condition.
• Must be able to drive an automobile in all types of weather conditions.
• Must be currently licensed in the state of employment.

Responsibilities

• Physiotherapists examine and treat patients with physical impairments using physical modalities.
• Aim to improve mobility, independence in self-care, and daily living skills.
• Work with Specialty Programs: Balance and Fall Prevention, Spine Safety, Total Hip and Knee Replacement, Total Shoulder Replacement.
• Administer skilled care to clients per physician orders and care plan.
• Educate family and health care team members; operate under Branch Director supervision.
