You will travel within Thrall, Thorndale, Rockdale and the west Hwy 79 area, delivering care from Monday to Friday, with a focus on quality outcomes and compassionate service.

Qualifikationen

• Master’s degree in physical therapy from a CAPTE-accredited program.
• Currently licensed as a physical therapist in the state of operation.
• CPR certification and valid driver’s license required.

Aufgaben

• Provides patient care and documents treatment in accordance with physician orders and agency policy.
• Performs patient evaluations and selects appropriate diagnostic materials.
• Informs the physician of assessment results and recommended interventions.
• Develops and documents treatment plans and establishes priorities.
• Educates and counsels patients and families on treatment and home programs.
• Presents patient information to physicians and keeps team informed of status and plan.
• Meets agency productivity standards and contributes to team conferences.
