Job summary

Goodwin Living in Alexandria, VA seeks a licensed Physical Therapist to evaluate patients at home health visits and develop plans of care with families and caregivers. You will collaborate with nurses, OT, speech, and aides to coordinate services and ensure safety and progress.

The role requires Virginia licensure, CPR, and the ability to guide and educate patients and families while maintaining Medicare-compliant documentation and case management.

Qualifications

• Degree in Physical Therapy from an accredited program.
• Current Virginia Physical Therapist licensure.
• CPR certification (adult).
• Passes competency assessment before patient care.

Responsibilities

• Perform comprehensive PT evaluations and treatments per physician referral.
• Open cases and complete Oasis assessments as needed.
• Assess home environment and coordinate with health team for services.
