Job summary

Qualifications

• Holds a current Physical Therapy License in Virginia.
• Graduated from an approved physical therapy program.
• One year of clinical experience.
• Current CPR certification.
• Excellent communication, organizational, and human relations skills.
• Able to function with minimal supervision.
• Able to manage multiple projects and changing priorities.
• Able to employ critical thinking skills.

Responsibilities

• Performs initial and ongoing physical therapy assessments to determine client functional level.
• Performs direct therapeutic physical therapy services as planned by Agency staff and physician.
• Evaluates equipment needs to increase client function.
• Records and reports client status and progress to physician and Agency staff.
• Teaches and supervises nurses, home health aides, caregivers, and clients for continuous therapy.
• Consults with Agency staff on goals and outcomes; participates in clinical conferences and in-services.
• Delivers services per standards of treatment effective for client conditions.
• Assists with plan-of-care development with physician collaboration.
• Consults on discharge planning with Nurse Case Manager and other leaders.
• Maintains clinical records promptly and comprehensively.
• Develops maintenance therapy programs for Agency staff and caregivers.
