Job summary

Choice Health at Home seeks a Physical Therapist to assess, plan, and deliver individualized rehabilitation programs in patients' homes in Peoria, AZ. The role aims to improve mobility, relieve pain, and restore function after illness or injury.

The ideal candidate is licensed, with experience in home health and CPR certification, and is ready to collaborate with a multidisciplinary team while following agency policies.

Qualifications

• Graduate of an accredited physical therapy program and licensure.
• Current, valid Physical Therapy license.
• One year of home health experience preferred.
• Current CPR certification.
• Valid driver’s license and auto insurance.
• HomeCare HomeBase experience preferred.

Responsibilities

• Perform and document initial evaluations to identify functional limitations.
• Develop and implement individualized therapy programs to improve physical function and independence.
• Monitor patient progress and adjust treatment plans to achieve outcomes.
• Educate patients and families on exercises, safe mobility, and recovery strategies.
• Collaborate with team members to ensure continuity of care.
• Follow infection control practices and universal precautions during care.
• Complete accurate, timely documentation within company guidelines.
• Support agency philosophy and administrative policies.
