Elara Caring is seeking a dedicated Physical Therapist to join our home health team. You will evaluate patients, develop and adjust treatment plans, and provide high-quality PT services in patients' homes while coordinating with physicians and caregivers.

Qualifications

• Graduate of an APTA-approved physical therapy program.
• Current, unrestricted PT license for the state of work.
• Minimum 1 year of healthcare facility experience.

Responsibilities

• Evaluate patients and develop individualized treatment plans.
• Provide PT services in the home setting.
• Monitor progress and adjust care plans as needed.
• Collaborate with physicians, caregivers, and the care team.
• Complete timely clinical documentation.
• Supervise therapy assistants and staff as appropriate.
